Though choir is a relatively very safe activity, please consent to the following:
I am comfortable with the instructors helping with very basic medical care (ie. a bandaid)
I understand that photos and videos may be taken at events, but will not be used without consent
I accept responsibility for my child up to the start time of the class, and as soon as the class ends
I agree to either stay for the duration of the class, or remain reachable and close enough that I could return quickly in case of emergency
Liability: I agree to not hold the choir responsible for any damages or injuries
